Schedule Change Request Form

Please submit a completed request below to the OVPL Executive Committee. The team requesting a change must submit this no later than one week (7 days before the match's date).

We do recognize there are extenuating circumstances that may affect the timeliness of the request. Please contact us immediately if there are any additional concerns or needs.

Teams submitting requests are not guaranteed a change from the OVPL Executive Committee, and each team must agree to the proposed Date, Time, or Venue change before submitting this request.

The OVPL will provide a decision within 24 hours.
